This patch introduces a new subclass SwiftASTContextForModules, which
is now owned by TypeSystemSwiftTypeRef and initialized lazily. The
TypeSystemSwiftTypeRef that corresponds to the scratch context is
still owned by SwiftASTContextForExpressions.
