Bump version of mermaid-filter
Add table_rules to getopt error check
Use extra spaces to omit example code from the sed hacks
Remove --dpi arguments (they weren't doing anything)
Set image widths manually as needed
Add Pandoc lua filter for centering Mermaid pdf images
Add Pandoc lua filter for drawing rules above and below source code blocks
Shrink source code font
Shrink and fade "Begin Informative Comment"
Put "Appendices" separator on its own page
