Telemetry enhancements

• Append app_name = MongoDB MCP Server to the connection string
• Include is_atlas property that specifies if the connection is targeting an Atlas cluster
• Include atlas_hostname property (if possible) which is the first resolved SRV hostname in case the connection is targeting an Atlas cluster.
