This adds a template for proposals focused on the Swift Testing project.

The template itself looks great, the main thing I think we need to decide on before merging this is your question in #2709 (comment)

The feature and API proposal template in this repo is being relocated to
swift-evolution. (See
swiftlang/swift-evolution#2709.) This removes
the file from this repo and updates the documentation Readme file
accordingly.
